$250.00 – 90 Minute Initial Session
$150.00-215.00 – 60 Minute Session
$125.00-150.00 – 45 Minute Session

Insurance
I work with clients, covered by a variety of insurance companies, and as an out-of-network provider. To determine if you have mental health coverage through your insurance provider, the first thing you should do is call them to ask about coverage.
Self-pay clients can receive statements (called superbills) upon request to seek reimbursement from their insurance company. In some cases, insurance companies may reimburse clients who have out – of – network benefits.
Contact Full Circle Counseling to see if your insurance plan is accepted.
Please bring your current insurance card and current state issued driver’s license/I.D. to your first session, as well as co-payment, co-insurance, or deductible, as applicable.
Services may be covered in full or in part by your health insurance or employee benefit plan.
Please check your coverage carefully by asking your insurance provider the following questions:
• Does my health insurance plan include mental health benefits?
• What are my out of network mental health benefits?
• How much does my insurance pay for an out-of-network provider?
• How many therapy sessions does my plan cover?
• Do I need authorization or is approval required from my primary care physician?
Payment
You have the option of making payment by cash, check, health savings (HSA) or credit card at the end of each session. If you are utilizing out-of-network benefits, I will provide a superbill at the end of each month which you can submit to your insurance company for reimbursement.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand by contacting Usha directly. Otherwise, you will be charged for the full rate of the session, excluding medical emergencies.
